While being my favorite book, Jose Saramago's Blindess is actually quite disturbing, especially the asylum part. It's dark and dirty. And he wrote it without punctuation (kinda annoying and disturbing) and names. :/

            "Gerald's Game" - Steven King

Although he's one of my favorite writers,
this book was disturbing to me.

Lord of the Flies, The Painted Bird, Animal Farm and so on are disturbing indeed, but to me, the most disturbing books are the ones containing memories of ordinary people who suffered a lot - so, for example, Voices from Chernobyl, War's Unwomanly Face or Beyond Human Endurance: The Ravensbrück Women Tell Their Stories.

I Spit On Your Grave by Boris Vian. It was really disturbing in many aspects.

In harms way I think the author was Doug Stanton. The book tells the true story of the men that sank in the US Indianapolis. Very sad indeed.
